City Forms

The following is a list of required City forms to be used when building within the City.

For building projects exempt from a building permit and corresponding City forms, refer to Building Permit Application Questions and Concerns.

The following two (2) City forms are used in combination when applying for a building permit during the building permit application process. For additional information regarding the building permit application, please refer to Building Permit Application Questions and Concerns.

The Design Review Board Application is required as part of the submittal package that is used by the City's Design Review Board during their review process. This review is required for all new construction, including additions and remodels that alter the building's exterior. The Variance Application (Board of Adjustment) form is required when applying for a zoning variance on current City zoning regulations, such as, building setbacks or lot coverage requirements.
